The weights (612.36g silver / 87.48g gold) come from the classical nisab definition of 20 mithqal of gold or 200 dirhams of silver. These figures are universally accepted across the four Sunni schools of thought.
Do I check the Nisab on my Zakat date?
Yes. The Nisab is checked on your Hawl date — the lunar anniversary of the day your wealth first exceeded the Nisab. If your wealth was above the Nisab for the full lunar year and is still above it on your Hawl date, Zakat of 2.5% is due on your total zakatable wealth.
It is not enough to be above the Nisab today — you must have been above it continuously (in most scholars' view) or at least at the start and end of your Hawl year.
